2025 was widely predicted to be the breakthrough year for AI agents. The reality has been more complicated. Many teams have experienced the same pattern: the demo works, the pilot launches, and then things break in production. An agent hallucinates a discount. Bad data leads to bad outputs. The project gets rolled back. What we're finding is that most of these failures aren't about the LLM itself. They're integration failures. As Andrej Karpathy put it, we have a powerful new kernel but no Operating System to run it properly.
